Our approach involves a collaborative model of care employing nurse practitioners and care coordinators who specialize in dementia management. We prioritize shared decision-making, addressing individual needs, providing caregiver education, and establishing connections with community resources. By adopting this approach, our aim is to support individuals in prolonging home-based living, managing dementia-related behaviors, and alleviating caregiver strain while reducing financial burden.
Our program includes regular check-ins with a nurse practitioner to assess functioning and address the needs of both the individual and the caregiver. These assessments contribute to the development of a personalized care plan, facilitating the preservation of cognitive and functional capacity. Additionally, we assign a dedicated nurse care coordinator who conducts monthly check-ins and provides support in achieving established goals.
As part of our commitment to comprehensive care, we offer priority access to a wide range of support services as well as professionals specializing in functional, psychological, psychiatric, and behavioral changes. Our program is typically covered by most insurance plans and participants always have the freedom to opt-out whenever they choose.
Dementia is a broad term used to describe a group of cognitive disorders characterized by a significant and progressive decline in cognitive functions, including memory, thinking, reasoning, language, and the ability to perform everyday activities. It's not a specific disease itself, but rather an umbrella term that encompasses several underlying conditions that cause these cognitive impairments. Dementia impacts a person's ability to function independently and can have profound effects on their quality of life and relationships. Dementia Support Complete supports the diagnosed person, family, and caregivers throughout the course of this diagnosis.
During your first visit, you can expect to engage in discussions about your loved one’s medical and psychiatric history, current health concerns, and any symptoms they might be experiencing as well as a full list of current and previous medications. Your nurse practitioner will likely conduct a comprehensive history and might recommend or request certain diagnostic tests or screenings from your primary care provider based on your individual circumstances.
Your first appointment is also a great opportunity to establish open communication with your caregiver team, ask questions, and clarify any uncertainties you might have about your loved one’s health. You will also create a Care Plan with your nurse practitioner which will help narrow down your individual and family’s goals and needs. Once you have established a clear set of initiating goals, you will be contacting by one of our nurse care coordinators who will follow you and your caregiver team as long as you need. These calls will be at least monthly, sometimes more, in order to help you maximize function, independence, and dignity.
